PCP Collaboration
Our mission is to ensure that your primary care provider always remains the “driver of your care”, even when their office is closed, no appointments available, or there are other barriers preventing you from visiting your PCP.
​
We bridge the gap between patients and their PCP by dispatching trained responders to your home who will utilize advanced remote examination technology to facilitate a telehealth session with the PCP and perform a thorough remote examination in real-time.
​
This will ensure that your PCP is always at the forefront of patient care, avoiding unnecessary Emergency Room visits, and safely evaluating and treating patients at home without compromising the appropriate level of care.
If your PCP is unavailable to join the telehealth visit, one of our highly trained providers will conduct the telehealth visit. After the visit is over, a detailed summary will be sent to your PCP to ensure proper continuation of care.

These challenges often result in avoidable Ambulance transports and Emergency Room visits, with patients being treated by providers who are unfamiliar with their overall health, because those are the only options available. This disrupts the continuity of care, as your PCP who is keenly familiar with your overall health is not involved in your care, and may result in poorer health outcomes.

Study after study has proven that patients with PCPs driving their care benefit from better management of chronic diseases, lower healthcare costs, and a higher level of satisfaction with better outcomes.

The Solution

To eliminate the barriers that are keeping patients from visiting their PCP’s, by expanding the PCP’s reach beyond the confinements of their office and their hours.​

First Aide’s responders will go to the patient’s home, take vitals, collect blood and specimen samples, conduct rapid testing, EKG’s and set up a telehealth visit with a First Aide provider, or your existing PCP (if available). ​

The provider will order labs and prescribe medications as needed.
PCP’s are no longer limited to seeing patients only in their office
Patients can access their PCP’s even after hours
The PCP is always kept up to date and involved - even when the patient is treated by a First Aide provider
PCP’s can monitor and care for their patients who are bed bound and cannot come to the office

How It Works

Request an appointment on our website, or call our call center- including after hours, holidays, and weekends.

We send a trained Responder to your chosen location at the time of your appointment

Directed by the Provider, the Responder takes vitals, cultures, specimen collection, EKGs, BGL, blood work etc.

You and the Responder will communicate with the Provider using telehealth technology. The Responder has equipment to connect devices for the Provider to evaluate in real time.

The Provider can now accurately diagnose and treat you, prescribe medications, and recommend follow-up care as needed.

A detailed report will be sent to your Primary Care Provider, to ensure continuation of care.
